Cypress Feline Rescue Adoption Requirements

*All Adoption Requirements Must be Met As Indicated in Order to be Considered for Adoption!

Please provide 2 References who do not live with you.

1. Please provide the Names, Ages and Phone Numbers of All the people living in your home.
2. Please provide the name and contact information for your Current Veterinarian (or your past Vet or the one you will be using for a new pet.) *Current pets must be up to date on Vaccines & Annual Check-ups (we must verify this).
3. If you Rent or own a Co-op or Condo, you must provide a Letter from Management (or a copy of your lease) indicating that Pets (Cats) are Allowed. *Please provide a Phone Number for the appropriate person as we must verify this.
4. If you own a house please provide a current Mortgage Statement with (same) Name & Address of Applicant.
5. Please provide a copy of your current Drivers License or State ID and Current Utility Bill with the same address you will be using on the application. (If you have not updated to a new address, you MUST do this prior to applying for adoption. It is fast and easy at any DMV office or online at http://dmv.ny.gov/. Sorry, NO exceptions to this rule.
6. You must be at least 23 yrs. old to adopt a cat or kitten.
7. You must fill out the Adoption Application Completely and with Accurate Information. If approved you will be notified to schedule an appointment for a Home Check. (This is to verify that the home is Pet Safe, with screens on windows, and needed cat supplies on hand, etc. We will explain this in further detail if your adoption application is approved.)

NOTE: please take your time reading and filling out application,  it must be filled out accurately and all questions answered,  incomplete or inaccurate applications may not be approved.
